What is the Living Well Program?

The Living Well Program was created to learn more about different ways to change our behaviors to better our health. Some examples of behavior change are in nutrition, tobacco use, alcohol use, drug use, mental health, exercise, and overall quality of life. We want to give you the help you need to better your health if you wish.

How can I be eligible for the program?

You may be eligible if you are:

  • 18 years or older
  • A patient at one of our partner clinics
  • Have an upcoming appointment with your provider.
  • Have a working cell phone with access to the internet.
What is expected of me once I join the program?

You will be randomized into one of the study groups. This means that you will be assigned to a group by chance, like flipping a coin.

You will be asked to complete:

  • A baseline survey.
  • Your upcoming clinic appointment
  • A 5-minute survey about your appointment
  • Three follow up surveys at 3-months, 6-months, and 12-months from today.
  • To provide four urine samples during each survey
  • And an exit interview

    In addition, you might be asked to:

  • Have a phone call with a health coach 2 weeks and 6 weeks after your clinic appointment.
  • And complete weekly brief surveys by text message or on a website on your computer or smartphone.
Is this program mandatory?

Your participation in the program is VOLUNTARY. If you decide not to be in this study, it will not affect your services at your clinic to which you are otherwise entitled. If you decide to participate, you can refuse to answer any question.

How long is the program?

If you enroll in the program, it will last 12 months.

What are the incentives of participating?

You will receive up to $325 in gift card payments to compensate for your time participating in this program: $25 for the initial assessment and attending your upcoming primary care appointment at your clinic, $25 for the first urine sample, $50 for the 3 month assessment after enrollment in the study, $25 for the second urine sample, $50 for the 6 month assessment, $25 for the third urine sample, $50 for the 12 month assessment, $25 for the fourth urine sample, and $50 for the exit interview.

For gift cards option, you have the choice of Visa, Amazon, Target, and Food for Less. We are currently in the process of expanding to have more physical gift cards option as well as cash option.

Does it cost money to participate in the program?

No participation in the program is free and for your participation, you can earn up to $325 in gift cards.

What is the purpose of the urine test kit?

The purpose of the urine sample kit is to check your health status compared to your answers on the questionnaires. The medical urinalysis results will only be used for research purposes and will not be disclosed to you, your doctor or to anyone else. The urine sample will be discarded within the hour it is collected.

The urine sample kit will be mailed to you. However, they may be offered to you in the clinic before your appointment if you have an in-person visit.

Can I drop out of the program at any time?

You can end your participation at any time. Ending your participation will not affect the care you receive at your clinic.

How many times can I complete the program?

You can only complete the program once.
